Why we ask about Indigenous identity: E-nangaabe-jig Health Services provides health services to members of First Nations, Inuit, and Métis communities. We ask this question solely to confirm eligibility for our services. We use this information for that purpose only, we do not share it for any other reason, and we do not use it to make decisions about you beyond determining whether you qualify for the services we offer.

Note on health information: If any of our forms ask for information about your health, medical history, symptoms, or care needs, that information is treated as personal health information and is handled with the additional safeguards required under PHIPA. Please do not include sensitive health details in general contact forms unless specifically requested.

b. Event and clinic booking information

We let you register for events, clinics, and appointment sessions. When you book, we collect:
